Can I change the gender marker on my Michigan ID/driver’s license to match my identity?

Can I change the gender marker on my birth certificate to match my identity?

Yes. To change the gender marker on your birth certificate, complete an Application to Correct or Change Michigan Birth Record. You will need a medical doctor to complete a medical affidavit that states you have had appropriate clinical treatment for transition to male or female. You will also need to pay a $50 fee by check or money order payable to the “State of Michigan.” It will take about six weeks for the Michigan Department of Health and Human Services to process the application.

Do I have to use the template medical affidavit produced by the Michigan State Registrar’s Office to change the gender marker on my birth certificate?

No. The template is available for people to use if they want to, but they don’t have to. Your medical doctor could decide to draft their own affidavit. The affidavit should say your doctor has treated you, or has reviewed and evaluated your medical history. It must also say that you have had appropriate clinical treatment for transition to male or female, or are in the process of transitioning.

Can I change the gender marker on my passport to match my identity?

Yes. To change your gender marker, you must complete Form DS-11, which is the general form to apply for a passport. You will also need all of the following documents:

  • ID that resembles your current appearance
  • Passport photo that resembles your current appearance
  • A medical certification from your doctor saying that you are having, or have had, “appropriate clinical treatment” for gender transition
  • Proof of legal name change, such as an Order Following Hearing on Petition to Change Name (if you are also asking for a name change on your passport)
  • Proof of U.S. citizenship (to learn more, read the section “Provide Evidence of U.S. Citizenship” on the Apply in Person page of the DOS website)

The fee for a new passport book with a changed gender marker is $110, payable to the U.S. Department of State. The fee for a new passport card is $30. With both a passport book and card, you must also pay a $35 execution (acceptance) fee to the acceptance facility.
